Understanding the PHQ-9 Depression Screening Tool

The Patient Health Questionnaire-9 (PHQ-9) is the standard nine-item depression module from the broader PRIME-MD diagnostic instrument. It scores each of the nine DSM-IV criteria for major depressive disorder on a scale from 0 to 3.

Score Interpretation Thresholds

  • 0 – 4 Points (Minimal): Indicates little to no depressive symptom burden over the evaluated 2-week period.
  • 5 – 9 Points (Mild): Suggests mild depressive symptoms that may benefit from monitoring, sleep optimization, and social support.
  • 10 – 14 Points (Moderate): Represents a common clinical threshold for initiating structured counseling or professional support.
  • 15 – 19 Points (Moderately Severe): Indicates significant depressive symptom impact warranting active pharmacological or psychotherapeutic intervention.
  • 20 – 27 Points (Severe): Reflects high-intensity depressive distress requiring comprehensive psychiatric evaluation.
