Advanced Wells Criteria for DVT
Clinical prediction rule for estimating the probability of deep vein thrombosis.
Active cancer
Treatment ongoing, within the last 6 months, or palliative
Paralysis, paresis, or recent plaster immobilization
Lower-extremity paralysis, paresis, or recent plaster immobilization
Recent immobilization or major surgery
Bedridden for 3 or more days, or major surgery within the previous 12 weeks requiring general or regional anesthesia
Localized tenderness along deep venous system
Tenderness along the distribution of the deep veins
Entire leg swollen
Calf swelling ≥3 cm
Measured compared with the asymptomatic leg
Pitting edema confined to symptomatic leg
Collateral superficial veins
Non-varicose superficial veins
Previous documented DVT
Alternative diagnosis at least as likely as DVT
If another diagnosis is equally or more likely to explain the symptoms

Interpretation
A Wells score of 1 point or less indicates that DVT is unlikely under the two-level Wells model. A score of 2 points or more indicates that DVT is likely.

Diagnostic testing such as D-dimer testing or compression ultrasonography should be determined according to the patient's clinical situation and applicable guidelines.
