About the SOFA Score
The Sequential Organ Failure Assessment (SOFA) score evaluates dysfunction across six organ systems: respiratory, coagulation, liver, cardiovascular, central nervous system, and renal function. Each system receives 0 to 4 points, producing a total score from 0 to 24.
The respiratory component uses the PaO₂/FiO₂ ratio. Lower ratios indicate greater respiratory dysfunction, with the most severe categories requiring respiratory support.
The coagulation component is based on platelet count. Lower platelet counts receive progressively higher SOFA points.
The liver component uses total bilirubin. Higher bilirubin concentrations correspond to higher liver SOFA scores.
The cardiovascular component considers mean arterial pressure and the use and dose of vasoactive medications such as dopamine, epinephrine, norepinephrine, and dobutamine.
The central nervous system component uses the Glasgow Coma Scale. A lower GCS corresponds to a higher SOFA component score.
The renal component uses serum creatinine and urine output. Severe oliguria can result in a high renal SOFA score even when the creatinine concentration alone would suggest a lower score.
SOFA Score Severity
The SOFA score ranges from 0 to 24. A higher score indicates greater overall organ dysfunction. The score is intended for clinical assessment and serial monitoring rather than as a standalone diagnosis.
SOFA Trend Tracking
Serial SOFA measurements can help show whether organ dysfunction is improving, worsening, or remaining stable. A rising score indicates increasing total organ dysfunction, while a falling score may indicate improvement. Individual component scores should also be reviewed because the total score can remain unchanged even when dysfunction shifts between organ systems.
